I hereby irrevocably agree and consent that you, the Taskforce On Waukegan Neighborhoods (TOWN), and your assigns, may use all or part of my interview/performance for your program.

You have the right to use my picture, silhouette and other reproductions of my likeness, performance, music and voice in connection with the program in which this interview/performance may be incorporated, and in any advertising media material promoting it.

You shall have all right, title and interest in any and all results and proceeds from said use or appearance.

The rights granted you are perpetual and include the use of this interview/performance in any medium. All or part of the program may be shown, including broadcast, satellite, and cable television, internet and videocassettes.

You are not obliged to make any use of this interview/performance or exercise any of the rights granted you by this release.

I hereby release and hold the Taskforce On Waukegan Neighborhoods (TOWN), and your assigns, harmless from any claims or liabilities of any kind arising out of or in connection with the making or the use of the Submitted Material and the use of my name, likeness or biography.

I understand and agree that I will not receive any compensation of any kind in connection with any use of the Submitted Material or any other rights granted by me hereunder and that the Taskforce On Waukegan Neighborhoods (TOWN), and your assigns, is relying on the rights granted herein.

I also represent that I am eighteen (18) years of age or older (or if I am under eighteen (18) years of age my legal guardian has signed below in order to consent to all of the foregoing).

I have read and understand the meaning of this release.
